Other Fees
File copying, form completion, and report writing are billed at the applicable hourly rate.
Cancelation Policy
To avoid a $45 cancellation fee, please provide a minimum of 24 hours’ notice for appointment changes or cancellations.
Exceptions may be made in cases of illness, provided notification is received by 9:00 AM on the day of the appointment.
Alternatively, the cancellation fee may be avoided by transferring your appointment to a friend or family member. Please note that last-minute appointment swaps must be with an existing patient of the clinic.
Attendance Policy
To help ensure consistent care and fair appointment availability for all patients, we kindly ask that at least 80% of scheduled appointments be attended as booked.
Frequent cancellations or rescheduling — even when appropriate notice is provided — can significantly impact scheduling and reduce appointment availability for other patients. Individuals with repeated missed or rescheduled appointments may lose the ability to pre-book future appointments.
